Other colleagues may have additional thoughts, but this indication is for delayed radiation injury and CMS/insurances do require documentation of dates of radiation as an eligibility criteria. Obtaining the records and adding them to this patient's records will be necessary to satisfy medical necessity requirements. In the event of an audit, these records will be needed.
The scope is helpful as is mention of delayed radiation injury.
