Hi I have a payer denial, BCBS of SC stating the HBO is investigational on the following patient's condition. It was preapproved with the codes below. I'm thinking this may be a coding issue. Would it help adding N32.89 or can you suggest something better?

This is a 60-year-old gentleman who has history of prostate cancer, treated with radical prostatectomy over a decade ago. The patient received radiation therapy for recurrent high levels of the PSA. The PSA had been undetectable.

So now the patient has been having bloody discharge from the penis for many months. He was referred to Urology who performed a cystoscopy. The cystoscopy showed irritation of the bladder neck at the 12 o'clock position with mild oozing, in the retro view of the bladder neck there was also petechia of the bladder neck (most likely related to the radiation therapy).

Referral to the Wound & Hyperbaric Clinic was R36.9 and C61. In the comments were, "Bleeding from the bladder neck after radical prostatectomy and radiation therapy."

BCBS of SC pre-approved HBO treatments with the following diagnoses: L59.8, R36.9, C61

BCBS of SC denied 99183 with L59.8, R36.9, C61
CODES:
{ 55 }- Procedure/treatment/drug is deemed experimental/investigational by the payer. Usage: Refer to the 835
Healthcare Policy Identification Segment (loop 2110 Service Payment Information REF), if present.
{ PR }-Patient Responsibility
{ N623 }-Not covered when deemed unscientific/unproven/outmoded/experimental/excessive/inappropriate.

Yet BCBS of SC paid 99183 with L59.8, K12.33, J35.8, C09.9 on another patient

Hi Kelly,

Thank you for the question. When coding radiation Cystitis with hematuria,  If you were already pre-approved with the first code being L59.8, then I would suggest adding N30.41. Please refer to the HBO crosswalk for STRN.

From here, if you continue to see denials for the 99183, you will need to appeal the claim with supporting documentation from your pre-approval.
