I have a 75 y/o M who had an extensive multiloculated soft tissue abscess in the lower lumbar and sacral subcutaneous tissue. This measured 15 x 6 cm transversely and 14 cm superior to inferior. He also had another area in the right lower paraspinal musculature that measured 2.9 x 1.4 cm transversely extending to sacrum and 6 cm superior to inferior. He had incision and drainage of the abscess with placement of penrose drain by general surgery. The penrose drain will be removed soon, but the two cruciate incisions are not very large and I'm hesitant to place a wound vac as visual field is limited. The wound measures 7.2 cm between the incisions and then extends down to right buttock nearly 5 cm. I'm looking for recommendations for packing options. Also wondering if the surgeon making a larger incision to be able to place a wound vac would be the better option.

Other colleagues might have other suggestions, sharing some thoughts here. Regarding recommendations for packing options, Dr Charash posted great insights in this curbside consult

As for having the surgeon make a larger incision to be able to place a wound vac - one thought to consider is that the use of NPWT in infected wounds requires careful clinical judgment and should be accompanied by appropriate antibiotic therapy and close monitoring for signs of infection. Also, it would be important to check if no contraindications are present (e.g. untreated osteomyelitis, unexplored fistulas, or exposed blood vessels). So, having the surgeon just make a larger incision, without proper blunt dissection of loculations, removal of pus/debris, or assessment of contraindications, would not be enough to provide adequate preparation prior to placement of NPWT. Alternatively, for patients in an inpatient setting where this resource is accessible and the patient does not have any contraindications, considering NPWT with the application of a cleansing solution (e.g., VeraFlo) could be an option.
